At HealthSpan Chicago we believe you should know exactly what you're paying for and exactly what you're getting. Here is everything you need to know about membership and consultation pricing.
Individual Membership — $5,000/year
Single member. Everything you need to take control of your long-term health — in one membership, with one physician who knows you. All visits, comprehensive testing, direct physician access, and care coordination included.
Spouse / Partner Add-On — $4,500/year ($9,500/year total)
The same comprehensive benefits extended to a spouse or partner. Same physician. Same access. Same commitment to your long-term health — together.
Family Membership — Contact Practice for Pricing
We offer family membership pricing for households that want comprehensive longevity care for the whole family. Please contact us directly to discuss your family's needs and we'll build a plan that works for you.
We understand that $5,000 upfront is a meaningful investment. That's why we offer four payment plan options to make membership accessible regardless of how you prefer to manage your finances.
NOTE: All fees are due on or before the first day of each billing period with a five business day grace period. Practice reserves the right to require automatic payment enrollment for installment plans. Payment may be made by credit card, ACH/bank transfer, check, or HSA/FSA card.
If you completed an initial consultation with HealthSpan Chicago prior to enrolling in membership within the same calendar year, a credit of $750 will be applied toward your first year's membership fee.
Membership at HealthSpan Chicago includes everything below — with no hidden fees, no surprise bills, and no co-pays.
Direct primary care membership fees may be eligible for payment using a Health Savings Account or Flexible Spending Account. We recommend confirming eligibility with your HSA/FSA administrator before enrolling.
While membership fees are not covered by insurance, many services outside of your membership — including labs beyond your annual panel, imaging studies, specialist referrals, and additional testing — are eligible to be billed through your health insurance. We will always be transparent about what is included and what may involve your insurance.
We require all members to maintain active health insurance coverage for specialist care, hospitalizations, emergencies, and services outside the scope of membership.
IMPORTANT NOTICE:
THIS MEMBERSHIP IS NOT HEALTH INSURANCE. Membership fees are a payment for Direct Primary Care services only.
Medicare does not cover Direct Primary Care membership fees. Members enrolled in Medicare (Part A, B, C, or D) may still use their Medicare benefits for services outside this membership — including specialist care, hospitalizations, and imaging. Members who are enrolled in Medicare will be required to complete a separate Medicare opt-out agreement. Please contact us directly if you have questions about Medicare and membership.
Beginning June 2026, HealthSpan Chicago will offer standalone consultations for individuals who want expert longevity and lifestyle guidance without committing to a full annual membership. These consultations are purely advisory and do not replace primary care or establish a primary care relationship.
Initial Consultation — $750
A comprehensive deep dive into your health history, goals, labs, and lifestyle. We build a personalized roadmap together with clear, actionable next steps. Available virtually and in person.
Follow-Up Consultation — $350
Available at the physician's discretion after your initial consultation. Focused sessions to track progress, adjust recommendations, and answer questions as you implement your plan.
Osteopathic Manipulative Therapy (OMT) — Same Rate Structure
Standalone OMT sessions for non-members follow the same consultation rate structure — an initial comprehensive appointment followed by shorter follow-up sessions at the physician's discretion. Please contact us directly for details on scheduling and availability.
If you complete an initial consultation and decide to enroll in membership within six months of that consultation, your $750 consultation fee will be credited toward your first year's membership fee. Follow-up consultation fees are not eligible for this credit.
